The Prevention and Support Service provide a high quality service to support the Social Services Wellbeing Act 2014 within a social care setting with children and young people.
By developing good working partnerships with them, their families, foster carers and other stake-holders, all with the aim of meeting identified needs.
To identify service need alongside the referring case holding social worker which will be beneficial to the children, young people, parents and carers within Wrexham County Borough or as near to this area as possible.
To promote and facilitate supervised contact with children, young people within the care system.
To promote and facilitate one to one direct family work identified by the case holding social worker within the Local Authority.
To promote and facilitate one to one direct work with children and young people ascertained by the case holding social worker within the Local Authority.
To promote social inclusion and prevent family breakdown by using appropriate considered interventions with children, young people, parents and carers.
